What is a blameless postmortem?

A blameless postmortem looks at the systems and conditions that allowed an incident to happen and leaves individuals out of it. People can then describe what they saw and did without worrying about consequences, which is how the real causes come out. It assumes everyone acted reasonably with what they knew at the time.
